Uninsured Services and Block Fees:

While most of your care is covered by the Ontario Health Insurance Plan (OHIP) certain medical and administrative services are not. These services are termed uninsured services and are charged directly to patients.

These cost of each service are listed below and are set in accordance with the guidelines set by the Ontario Medical Association (OMA) and the College of Physicians and Surgeons of Ontario (CPSO).

You can opt to pay-per service or consider a Block Fee (see below for details).

The clinic reserves the right to update the list and cost of uninsured services.

BLOCK FEE

In an effort to reduce administrative costs, and provide cost savings to patients, we offer a Block Fee program. The Block Fee Program provides 365 days of coverage for most services (see below for inclusions and exclusions) from the date of purchase. The Block Fee Program is entirely optional. For those that do not wish to subscribe, charges will be pay-per service.

Block Fee Program rates are:

• Senior (age 65 or older)/Student/Individual on Income Assistance: $175

• Individual: $195

REFUNDS

A refund on a purchased Block Fee is offered up to 60 days after the date of purchase. Please note that if you received any uninsured services during this period and subsequently cancel your Block Fee, that we will refund you the Block Fee minus the amount for services already provided .

No refunds are available on charges for individual services once provided.
